    The incorporation of carbon nanofibres to enhance the properties of hot compacted self-reinforced single polymer composites

    Nanoscale fillers offer the potential for significant enhancement of a range of polymer properties, as they are available in a wide variety of shapes and properties. Carbon nanotubes (CNT) and nanofibres (CNF) have been used extensively in the literature, yet very few analytical studies of the material properties have been reported. Here we use the Cox-Krenchel model to interpret the experimentally measured changes in Young’s modulus from particle aspect ratio reduction during to processing, in addition to the measurement of the mechanical properties of the composite. Hot Compaction, a process developed at the University of Leeds [1], utilises high modulus, highly oriented elements to form thick section, homogeneous sheets without the need to introduce a second phase of different chemical composition. These ‘single polymer’ composites are produced by selective melting on the surface of the oriented elements; on cooling, this molten material re-crystallises to form a matrix phase and bind the oriented elements together. CNF filled polypropylene (PP) tapes have been produced and successfully hot compacted into sheets. The properties of these nanofilled self-reinforced single polymer composites is reported. Of particular interest has been to investigate the introduction of interleaved films, an extension of recent work conducted by two of this papers authors [2] of the same polymer or nanocomposite in order to establish the change in properties when the CNF are incorporated in the drawn tapes, in the interleaved films or both

    The potential application of artificial intelligence for diagnosis and management of glaucoma in adults

    BACKGROUND: Glaucoma is the most frequent cause of irreversible blindness worldwide. There is no cure, but early detection and treatment can slow the progression and prevent loss of vision. It has been suggested that artificial intelligence (AI) has potential application for detection and management of glaucoma. SOURCES OF DATA: This literature review is based on articles published in peer-reviewed journals. AREAS OF AGREEMENT: There have been significant advances in both AI and imaging techniques that are able to identify the early signs of glaucomatous damage. Machine and deep learning algorithms show capabilities equivalent to human experts, if not superior. AREAS OF CONTROVERSY: Concerns that the increased reliance on AI may lead to deskilling of clinicians. GROWING POINTS: AI has potential to be used in virtual review clinics, telemedicine and as a training tool for junior doctors. Unsupervised AI techniques offer the potential of uncovering currently unrecognized patterns of disease. If this promise is fulfilled, AI may then be of use in challenging cases or where a second opinion is desirable. AREAS TIMELY FOR DEVELOPING RESEARCH: There is a need to determine the external validity of deep learning algorithms and to better understand how the 'black box' paradigm reaches results

    Has the EAGLE landed for the use of clear lens extraction in angle-closure glaucoma? And how should primary angle-closure suspects be treated?

    Angle-closure glaucoma is an aggressive condition that causes millions to become blind worldwide. This review explores the use of prophylactic laser peripheral iridotomy (PI) in patients classified as primary angle-closure suspects (PACS), and additionally, the use of clear lens exchange as a primary treatment option in established angle-closure disease with or without glaucoma. As PI has a strong prophylactic effect in fellow eyes of patients who have had an acute attack, its use has been widely adopted in those patients classified as PACS, but with limited evidence to support this. A large randomised trial conducted in China has demonstrated that although PI reduces the risk of incident angle-closure disease, the incidence of disease that would threaten vision was much lower than anticipated. This suggests that the benefit of prophylactic PI is very limited. Health services data shows an association between rising cataract surgical rates and of decreasing rates of acute angle-closure. Age-related growth of the lens is a major component of angle-closure disease. Several studies have shown that clear lens extraction (CLE) effectively lowers IOP in angle-closure. The use of CLE as a primary treatment option has been tested against LPI in the EAGLE study, a large RCT that enroled people with angle-closure and an IOP > 30 mmHg, and those with angle-closure glaucoma. The trial showed CLE to be superior to PI both for IOP control and patient reported quality of life. On these grounds, CLE should be considered for first-line treatment of more advanced angle-closure disease

    Virtual clinics in glaucoma care: face-to-face versus remote decision-making

    BACKGROUND/AIMS: To examine the agreement in clinical decisions of glaucoma status made in a virtual glaucoma clinic with those made during a face-to-face consultation. METHODS: A trained nurse and technicians entered data prospectively for 204 patients into a proforma. A subsequent face-to-face clinical assessment was completed by either a glaucoma consultant or fellow. Proformas were reviewed remotely by one of two additional glaucoma consultants, and 12 months later, by the clinicians who had undertaken the original clinical examination. The interobserver and intraobserver decision-making agreements of virtual assessment versus standard care were calculated. RESULTS: We identified adverse disagreement between face-to-face and virtual review in 7/204 (3.4%, 95% CI 0.9% to 5.9%) patients, where virtual review failed to predict a need to accelerated follow-up identified in face-to-face review. Misclassification events were rare, occurring in 1.9% (95% CI 0.3% to 3.8%) of assessments. Interobserver Îș (95% CI) showed only fair agreement (0.24 (0.04 to 0.43)); this improved to moderate agreement when only consultant decisions were compared against each other (Îș=0.41 (0.16 to 0.65)). The intraobserver agreement Îș (95% CI) for the consultant was 0.274 (0.073 to 0.476), and that for the fellow was 0.264 (0.031 to 0.497). CONCLUSIONS: The low rate of adverse misclassification, combined with the slowly progressive nature of most glaucoma, and the fact that patients will all be regularly reassessed, suggests that virtual clinics offer a safe, logistically viable option for selected patients with glaucoma

    Correlation between geology and concentration-volume fractal models: significance for Cu and Mo mineralized zones separation in the Kahang porphyry deposit (Central Iran)

    ArticleThis study identifies the major mineralized zones including supergene enrichment and hypogene enrichment in the Kahang Cu-Mo porphyry deposit which is located in Central Iran based on subsurface data and utilization of the concentration-volume (C-V) fractal model. Additionally, a correlation between results achieved from a C-V fractal model and geological models consisting of zonation, mineralography and alteration have been conducted in order to have an accurate recognition and modification of the main mineralized zones. Log-log plots indicate five geochemical populations for Cu and Mo in the deposit which means that mineralization commences with 0.075 % and 13 ppm for Cu and Mo (as the first thresholds) respectively. The main mineralization began for Cu≄0.42 % and Mo≄100 ppm and also enriched mineralization containing Cu≄1.8 % and Mo≄645 ppm which is located in the central part of the deposit. According to the C-V model, the main Cu-Mo mineralized zones occur in the hypogene zone, especially in the central, NW and NE parts of the Kahang deposit. The supergene enrichment zone derived via the C-V model is smaller than that in the geological model and is located in the central and eastern parts of the deposit. Results analysed by the C-V fractal model certify that the interpreted zones based on the fractal model are accurate. To certify this, a logratio matrix has been employed to validate the C-V fractal model for the Cu and Mo main mineralized zones.The authors are grateful to the National Iranian Copper Industries Co.     Application of Polyethylene Glycol to Promote Cellular Biocompatibility of Polyhydroxybutyrate Films

    Polyhydroxybutyrate (PHB) is a biomaterial with potential for applications in biomedical and tissue engineering; however, its brittle nature and high crystallinity limit its potential. Blending PHB with a variety of PEGs produced natural-synthetic composite films composed of FDA-approved polymers with significant reductions in crystallinity, from 70.1% for PHB films to 41.5% for its composite with a 30% (w/w) loading of PEG2000. Blending also enabled manipulation of the material properties, increasing film flexibility with an extension to break of 2.49±1.01% for PHB films and 8.32±1.06% for films containing 30% (w/w) PEG106. Significant changes in the film surface properties, as measured by porosity, contact angles, and water uptake, were also determined as a consequence of the blending process, and these supported greater adhesion and proliferation of neural-associated olfactory ensheathing cells (OECs). A growth rate of 7.2×105 cells per day for PHB films with 30% (w/w) PEG2000 loading compared to 2.5×105 for PHB films was observed. Furthermore, while cytotoxicity of the films as measured by lactate dehydrogenase release was unaffected, biocompatibility, as measured by mitochondrial activity, was found to increase. It is anticipated that fine control of PEG composition in PHB-based composite biomaterials can be utilised to support their applications in medicinal and tissue engineering applications.     Optical Coherence Tomography in the UK Biobank Study - Rapid Automated Analysis of Retinal Thickness for Large Population-Based Studies

    PURPOSE: To describe an approach to the use of optical coherence tomography (OCT) imaging in large, population-based studies, including methods for OCT image acquisition, storage, and the remote, rapid, automated analysis of retinal thickness. METHODS: In UK Biobank, OCT images were acquired between 2009 and 2010 using a commercially available “spectral domain” OCT device (3D OCT-1000, Topcon). Images were obtained using a raster scan protocol, 6 mm x 6 mm in area, and consisting of 128 B-scans. OCT image sets were stored on UK Biobank servers in a central repository, adjacent to high performance computers. Rapid, automated analysis of retinal thickness was performed using custom image segmentation software developed by the Topcon Advanced Biomedical Imaging Laboratory (TABIL). This software employs dual-scale gradient information to allow for automated segmentation of nine intraretinal boundaries in a rapid fashion. RESULTS: 67,321 participants (134,642 eyes) in UK Biobank underwent OCT imaging of both eyes as part of the ocular module. 134,611 images were successfully processed with 31 images failing segmentation analysis due to corrupted OCT files or withdrawal of subject consent for UKBB study participation. Average time taken to call up an image from the database and complete segmentation analysis was approximately 120 seconds per data set per login, and analysis of the entire dataset was completed in approximately 28 days. CONCLUSIONS: We report an approach to the rapid, automated measurement of retinal thickness from nearly 140,000 OCT image sets from the UK Biobank. In the near future, these measurements will be publically available for utilization by researchers around the world, and thus for correlation with the wealth of other data collected in UK Biobank. The automated analysis approaches we describe may be of utility for future large population-based epidemiological studies, clinical trials, and screening programs that employ OCT imaging
